Staffordshire and Stoke-on-Trent Integrated Care System incorporates Docobo’s remote monitoring functionality into One Health and Care

Staffordshire and Stoke-on-Trent Integrated Care System has incorporated Docobo’s remote monitoring capability into its One Health and Care (OHC) platform. This will enable clinicians to access data about patients registered at any GP practice in Staffordshire or Stoke-on-Trent who are being remotely monitored. If a patient registered at one of these GP practices seeks services at any organisation outside of Staffordshire or Stoke-on-Trent – but within the OHC scope – e.g. Walsall Manor Hospital – the user will have access to the remote monitoring information for this patient.

This will help support clinical decision making about patients, as clinicians have instant access to baseline readings from patients’ latest remote monitoring observations, which may include temperature, blood oxygen level, blood pressure and any recorded symptoms.

One Health and Care is a confidential digital shared care record for people living in Staffordshire and Stoke-on-Trent and brings together data from across health and social care. It allows registered health and social care professionals to view relevant information in order to provide better, safer care. The solution is already being used for a small number of patients.

Following this initial implementation of the system in Staffordshire and Stoke-on-Trent, a pipeline of pathways and populations will shortly be onboarded.
