Overview of Northamptonshire virtual hub project

Northamptonshire – Remote monitoring service overview

Phil from Docobo, introduces the Northamptonshire project that uses a virtual hub to help support vulnerable people accessing community services.

Pedro Santos (Integrated Care Northamptonshire) and Paula Love (Northamptonshire Healthcare Foundation Trust) review the project so far:

  • Why they chose Docobo for their remote monitoring solution.
  • What their initial motivations and challenges were.
  • What benefits have been evident since beginning the project.
  • How they have measured their successes.

Giles West, Deputy Director of operational transformation for Northamptonshire healthcare, announces the five year ambition of the project is to have 10,000 people living in their own homes with remote monitoring, being supported via the hub with wrapped around support from the integrated neighbourhood teams. He then offers deeper insight as to the positive impact that this level of growth could promise for the future.

This project demonstrates the strength of the partnership between health and social care services, how they can successfully work together to integrate better care and wrap services around individuals to help them live well, stay well and age well.
