Remote monitoring in Rugby care homes

Carol Judd-Winn, Lead District Nurse in Rugby, reveals her experience of remote monitoring used within the homes that she supports; these include learning disabilities, care homes and nursing homes. Carol discusses three pathways:

Pathway one: Virtual ward rounds – residents’ health information is added at the time it is observed. Clinicians can then review all the relevant information during ward round and make better informed decisions.

Pathway two: Monthly wellness checks – carers submit resident readings at regular intervals which are reviewed by clinicians. Carol observes that these have demonstrated a huge reduction in falls within the care homes.

Pathway three: My resident is unwell – carers use this pathway to replace needing to phone the GP when they are concerned about a resident. Carers submit their concerns from their device and will receive a response within two hours, offering advice and/or an appropriate intervention for the resident (eg a visit from GP or relevant clinician)
